Job summary

Amity Foundation in Moreno Valley, CA seeks a Nurse Care Manager for the Enhanced Care Management (ECM) Community Supports program. You will oversee a caseload of members with complex medical needs, perform medication reconciliations, educate families, and coordinate care with the ECM team to reduce hospitalizations and improve outcomes.

The role requires an RN or LVN license in California, 5+ years nursing experience (2+ in care management or related field), and willingness to travel locally.

Qualifications

• RN or LVN licensed by the California Board of Registered Nursing.
• Minimum 5 years nursing experience, with at least 2 years in care management or related field.
• Expert knowledge of healthcare practices, care coordination, and patient-centered care.
• Willingness to travel locally as needed to support program initiatives.

Responsibilities

• Supports ECM Members with complex medical conditions and completes medication reconciliation with pharmacy collaboration.
• Manages a caseload of ECM-enrolled Members (primarily Tier 1 and 2).
• Engages Members to activate and achieve health goals.
• Uses evidence-based communication strategies to discuss responsibilities and coordinate tasks.
• Tracks health outcomes in the care management platform and ensures data accuracy.
