Oak Hill is hiring a Direct Support Professional (Part-Time, Day Shift) in Hebron, CT. You will provide person-centered support in a residential setting to individuals with intellectual and developmental disabilities, helping them live more independently and participate in community life.

The role includes assisting with daily living activities, health and wellness monitoring, meal preparation, and documentation in the electronic health record.

Qualifications

• Must be at least 18 years of age.
• Valid driver’s license with ability to transport individuals in agency vehicles.
• Ability to successfully complete background checks, fingerprinting, and pre-employment health screenings.
• Ability to complete required training programs and maintain necessary certifications.
• Commitment to maintaining a safe, healthy, and professional work environment in accordance with Oak Hill policies.
• Commitment to continuity of care, including an occasional need to stay beyond a scheduled shift until coverage is in place.

Responsibilities

• Support individuals in living healthy, meaningful, and fulfilling lives by assisting with daily living activities.
• Monitor and document health-related information and administer medications per training and directives.
• Transport individuals to appointments and community activities using agency vehicles.
• Prepare meals and snacks according to menus and dietary requirements; provide mealtime support.
• Assist with mobility and transfers using approved techniques and equipment.
• Document daily activities, health information, progress notes, incidents, and medication administration in the EHR; follow regulatory requirements.
• Respond to emergencies and behavioral needs using established procedures and safety protocols, including First Aid/CPR when required.
• Communicate effectively with colleagues, individuals served, families, and other stakeholders; participate in trainings and meetings.
