Oak Hill in Glastonbury, CT is seeking a Direct Support Professional for the evening shift (part-time) to assist individuals with intellectual and developmental disabilities in a residential setting. You will support daily living activities, health monitoring, meals, mobility, and community engagement while documenting care in the EHR.

Qualifications

• Must be at least 18 years old.
• High school diploma or GED.
• Valid driver's license with reliable transportation.
• Ability to complete background checks and health screenings.

• Willingness to stay beyond shift if needed.

Responsibilities

• Assist with daily living activities (grooming, hygiene, dressing).
• Provide continence care as needed.
• Encourage independence and skill development.
• Monitor and document health information; assist with medications per training.
• Transport individuals to appointments and activities.
• Prepare meals and assist during mealtimes.
• Assist with mobility and transfers using proper techniques.
• Document in the electronic health record (EHR).
• Follow health care plans and safety protocols.
• Collaborate with team and participate in trainings.
