Job summary

LEL Home Services, LLC seeks a Direct Support Professional to provide hands-on care to assigned individuals in their homes. You will follow the ISP, maintain accurate records, and communicate with the Program Manager and families to coordinate schedules and services.

You will assist with daily living activities, encourage participation in community activities, and ensure a safe, respectful environment. Evening, weekend, and flexible hours are required as assigned.

Qualifications

• Meets all Direct Support Professional Requirements and Criminal History and Background Policy requirements.
• Completes all required training as directed by LEL and LEL's Training Procedure.
• Committed to serving individuals with developmental and intellectual disabilities.

Responsibilities

• Implement the services outlined in their respective ISP.
• Submit accurate time records at end of each shift.
• Notify Program Manager of changes in daily schedules.
• Provide support in activities of daily living per NOA.
• Ensure a safe environment in the Individual's residence.
