Job summary

Qualifications

• High School Diploma required.
• BA/BS in sociology, psychology, social work, or related field preferred.
• Experience in community-based outreach and health education preferred.
• Healthcare and health insurance knowledge preferred.
• Bilingual English/Spanish preferred.

Responsibilities

• Provide in-person community-based outreach and engagement in local settings including the emergency department.
• Provide integrated case management support addressing housing, insurance, medical care, transportation, and appointment attendance.
• Assist in access to care and connect patients with Hartford HealthCare and CHH resources.
• Collaborate with medical, nursing, and ancillary staff to identify and remove barriers to care.
• Assist Care Manager with coordination of patient care in the community, during hospitalization, and at discharge.
• Meet customers in the emergency department to link them with community outreach services.
• Advocate for patients facing barriers to care including language, transportation, insurance, or scheduling issues.
• Attend appointments with customers to provide support and linkages to other providers.
• Assist customers in completing benefits applications and state assistance paperwork.
• Perform other related duties as required.
