Generations Family Health Center in Willimantic, CT is seeking a Community Health Worker to screen individuals, share program information, and connect clients with community resources. You will market the Healthcare to the Homeless program and attend local meetings to build collaboration with community sites.

Responsibilities include coordinating referrals, providing health education, and assisting with outreach to targeted populations.

Qualifications

• Associate’s degree in human services or related field, or equivalent related course work and work experience desired.
• Minimum of two (2) years experience with logistics coordination and outreach/case management, preferably in a health care setting.
• Valid driver’s license and availability to properly insured vehicle throughout employment.
• Bilingual candidate (Spanish/English) preferred based on patient population.

Responsabilités

• Screen individuals or families using a standardized tool.
• Provide educational materials and community referrals as needed.
• Market Healthcare to the Homeless program to the community and attend meetings and events.
• Develop and maintain relationships with community sites and programs.
• Coordinate administrative functions to ensure smooth patient flow and high quality service.
• Provide health outreach, education and support services to targeted populations.
