Job summary

Paycom - ATS in Connecticut seeks a Case Manager – Certified Community Health Worker to support supportive housing programs. You will perform outreach, engagement, education, coaching, and care coordination, with a focus on social determinants of health and client-centered planning.

Responsibilities include universal screening, ISP development, referrals to clinics and community supports, documentation in STEPS/HMIS, and monthly home visits.

Qualifications

• Bachelor’s degree required; education may be substituted with experience on a year-for-year basis.
• Two (2) years of experience providing direct client services to populations experiencing substance abuse, homelessness, and/or mental illness.
• Must have a CT Community Health Worker certificate or be able to obtain within six months of hire.

Responsibilities

• Identify and engage customers in the community; link them with available services.
• Interview customers on-site or off-site; complete universal screening for eligibility.
• Develop individualized service plans (ISP) and coordinate referrals to clinics and community supports.
• Coordinate/monitor services; document ISPs and outcomes; maintain client records per requirements.
• Conduct monthly home visits and bi-weekly check-ins; update STEPS/HMIS systems.
• Provide advocacy and mediation to address customer needs and referrals; participate in team meetings.
