Job summary

The position collaborates with behavioral health, primary care, dental and other UCFS services to empower clients, conduct outreach, and provide mentoring. Travel up to 25–30% may be required.

Qualifications

• Associate in Human Services or related field
• 1 year of experience working with community resources
• 1 year of experience in the generation of care plans for clients or patients
• Community Health Worker Certification
• Ability to travel 25-30% of a work schedule
• Bilingual Spanish or Haitian Creole
• Proficiency with EPIC EHR and Microsoft Office Suite

Responsibilities

• Provides initial and ongoing tele-phonic and in-person support to clients seeking assessment for social determinants of health (SDOH).
• Collaborates with health services to develop a patient-centered care plan.
• Provides peer mentoring, coaching, and guidance related to consumer needs.
• Establishes and facilitates a client communication plan and follow up.
• Maintains familiarity with community resources and connects clients to services.
• Conducts outreach to targeted groups to address barriers to access.
• Performs related administrative duties and documentation.
• Organizes awareness events and speaking engagements.
