Talentify, in partnership with Hunterdon Health, seeks a Community Health Worker to improve access to care and health outcomes in the Hunterdon Health service area. The CHW bridges clinical settings and communities, addressing social determinants of health and linking patients to preventive services, screenings, medications, and local resources.

The role emphasizes outreach, case management, navigation, and collaboration with the health team to reduce barriers and monitor progress in uninsured

Qualifications

• Strong written and verbal communication skills and ability to speak to groups of varying sizes.
• Proficient computer skills including Google Workspace and Microsoft Office (Excel, Word, PowerPoint).
• Strong organizational skills and ability to work independently or as part of a team.
• Valid driver’s license and reliable transportation.
