Responsibilities

• Conduct comprehensive assessments of clients' physical, psychological, social, and environmental needs.
• Implement individualized care plans in collaboration with clients, families, and other stakeholders.
• Coordinate and monitor the delivery of services to ensure timely care.
• Advocate for clients' rights and assist them in accessing resources.
• Provide emotional and practical support to clients and families.
• Regularly evaluate progress and adjust care plans as needed.
• Maintain accurate documentation of care, progress and outcomes.
