Job summary

Family & Children’s Services in Tulsa is seeking a Certified Behavioral Case Manager to provide wrap-around case management for adults facing homelessness and mental health challenges. You will work within a CCBHC framework to coordinate services, referrals, and community resources, ensuring person-centered care.

The role emphasizes trauma-informed practices, multi-disciplinary teamwork, and ongoing documentation of client progress to support recovery and stability.

Qualifications

• Bachelor’s degree in social sciences preferred.
• Case Management Certification or obtain by hire.
• Must have a valid Driver License.
• Skills to build supportive client relationships with mental illness/substance use.
• Experience working with adults who are homeless or housing unstable.

Responsibilities

• Attend and participate in daily treatment team meetings with multidisciplinary staff.
• Document client progress to maintain a permanent record.
• Coordinate wrap plans and treatment plans using a strengths-based approach.
• Link clients to housing, social services and community resources.
• Respond to crises and provide trauma-informed interventions.
• Coordinate care with external health providers and referrals.
• Ensure timely transfer of medical records and follow-up steps.
