Job summary

Qualifications

• HS diploma or GED required; bachelor's degree preferred.
• Experience in mental health, criminal justice, or community services needed.
• Ability to work with diverse clients and families.

Responsibilities

• Conduct comprehensive assessments of clients' physical, psychological, social, and environmental needs.
• Develop individualized care plans with clients and families to promote reintegration into the community.
• Coordinate and monitor service delivery to ensure timely care.
• Advocate for clients' rights and help access resources and services.
• Provide emotional and practical support to clients and families.
• Evaluate progress and adjust care plans as needed by program.
• Maintain accurate documentation of care, progress, and outcomes.
• Facilitate communication among multidisciplinary teams and participate in case conferences.
• Educate clients and families about available services and assist in decision making.
• Ensure compliance with laws, regulations, and standards; engage in ongoing education.
