Qualifications

• High school diploma or GED required.
• Bachelor's degree preferred.
• Experience in mental health, criminal justice, or community-based social services required.
• ODMHSAS Case Manager II Certification within 60 days of eligibility.
• Valid Driver License and satisfactory driving record for client transport.

Responsibilities

• Conduct comprehensive assessments of clients' physical, psychological, social, and environmental needs.
• Implement individualized care plans in collaboration with clients, families, and stakeholders to promote reintegration.
• Coordinate and monitor the delivery of services to ensure timely and appropriate care.
• Advocate for clients' rights and assist in accessing resources.
• Provide emotional and practical support to clients and families.
• Maintain accurate documentation of care, progress, and outcomes.
• Facilitate communication among multidisciplinary teams and participate in case conferences.
• Educate clients and families about available services and resources.
