Job summary

NorthCare seeks a Behavioral Health Case Manager II to provide outpatient care navigation for adults, children, and families in Oklahoma. You will coordinate physical and behavioral health services, engage clients as partners in their own care, and work within ODMHSAS criteria to ensure quality, accessible care across settings.

Role requires community-based work, documentation, travel within the service area, and ongoing professional development including certifications and trauma-informed

Qualifications

• Must meet ODMHSAS Case Management II criteria.
• Minimum 1 year experience in mental health or healthcare setting preferred.
• Must obtain ODMHSAS Case Management certification within 60 days of hire.

Responsibilities

• Provides integrated services addressing physical and behavioral health and social needs.
• Promotes trauma-informed, patient-centered, recovery-focused care.
• Collaborates with a multi-disciplinary team to support health behavior change.
• Conducts assessments and develops care management plans; coordinates benefits and housing resources.
• Provides crisis management, safety planning, and intervention as needed.
• Maintains client records and documents interactions promptly and accurately.

High school diploma or GED with 36 months direct consumer care experience

60 college credit hours with 12 months direct consumer care experience
