Job summary

Family & Children's Services is seeking a Case Manager to coordinate and provide client-centered care across home, community, and facility settings in the OKC area. You will assess needs, develop care plans, coordinate services, and advocate for clients' rights and resources to support optimal well-being.

The role involves collaborating with multidisciplinary teams, maintaining documentation, and adhering to applicable laws and standards. Evening/weekend hours may be required as needed.

Qualifications

• High school diploma or GED required; Bachelor's degree preferred.
• Experience in mental health, criminal justice, or community social services required.
• ODMHSAS Case Manager II Certification within 60 days of eligibility.

Responsibilities

• Assess clients' physical, psychological, social, and environmental needs.
• Develop and implement individualized care plans with clients and families.
• Coordinate and monitor services to ensure timely care.
• Advocate for clients' rights and access to resources.
• Document care progress and outcomes accurately.
• Support multidisciplinary teams and participate in case conferences.
