Family & Children’s Services is seeking a Case Manager for the Adult Outpatient Connections program in Tulsa. You will manage a caseload, coordinate services, and advocate for clients to achieve optimal health and independence.

Qualifications

• Bachelor's degree fields accepted; experience in behavioral health preferred.
• Experience in mental health, criminal justice, or community-based services required.

Responsibilities

• Coordinate and provide client-centered care.
• Assess needs and develop care plans; advocate for client rights.
• Conduct in-home, community or facility-based services; offer crisis intervention when needed.
• Educate clients or groups on money management, symptom education, and daily living.
