Qualifications

• Bachelor’s degree or higher required for some CM levels with related experience in behavioral health.
• Case Manager IV requires a Bachelor’s in Social Science or related field.
• Case Manager II requires HS diploma or equivalent plus specified behavioral health experience or college credit.

Responsibilities

• Coordinate and provide client-centered care across home, community, or facility settings.
• Assess needs, develop care plans, coordinate services, and advocate for clients’ rights.
• Educate clients individually or in groups on topics like money management, symptom education, and relationships.
• Provide crisis intervention and manage caseload variety from monthly to weekly contact.
• Deliver services in-person or via telehealth and gather data metrics as required.
