Job summary

VOALA is seeking a Clinical Case Manager I in Los Angeles to deliver Intensive Case Management for clients with multiple co-morbidities and to coordinate services, counseling, and treatment planning for successful re-entry.

Under supervision, you will conduct screenings, develop individualized case plans, connect clients to resources, advocate with agencies, and participate in outreach. VOALA offers medical, dental, vision, and retirement benefits, with on-site work required daily.

Qualifications

• Fingerprint clearance and background check required.
• Valid California driver’s license and reliable transportation.
• Ability to work under emotional stress with challenging clients.

Responsibilities

• Conduct comprehensive screenings and assessments to collect information and develop a case plan.
• Coordinate individualized planning and service linkages to meet client needs.
• Coordinate case conferences, monitor progress, and advocate for clients with external agencies.
• Provide client advocacy before judicial, community, social service, and administrative bodies.
• Network with referral agencies and natural supports; participate in outreach events.
• On-site duties and reporting to work as required.
