Job summary

VOALA is seeking a Case Manager (Specialist) to provide intensive on-site case management to eligible clients, meeting participants weekly and coordinating a broad range of services. The role focuses on developing plans and linking clients to resources while advocating for their needs within community and social service systems.

Requirements include a relevant degree, at least 1 year in case management, and the ability to pass fingerprint/background checks.

Qualifications

Associate’s Degree in Social Work or related field.
Minimum 1-year general experience in case management or similar social services.
Ability to pass fingerprint clearance and background check.
Completion of VOALA Case Management training within 6 months of hire.

Responsibilities

Conducts comprehensive screenings and assessments to collect relevant information and develop a case plan.
Provide service linkages and support systems to ensure identified needs are met.
Coordinates individualized planning with clients to meet short- and long-term needs.
Coordinates case conferences with residents and monitors progress of case plans.
Acts as advocate for client before judicial, community, social service, and admin bodies.
Facilitates group education sessions and documents sessions clearly.
