Job summary

Volunteers of America Los Angeles seeks a dedicated Case Manager (Specialist) to deliver intensive case management to eligible clients, meeting participants weekly and coordinating services. The role requires strong assessment, planning, and advocacy skills in a on-site environment.

Responsibilities include conducting screenings, developing case plans, coordinating care, and advocating for clients within the community.

Qualifications

• Associate’s Degree in Social Work or related field required.
• Completion of VOALA Case Management training within 6 months of hire.
• Minimum 1-year general experience in case management or similar social services role.

Responsibilities

• Conducts comprehensive screenings and assessments to collect required information and develop a case plan.
• Provide service linkages and support systems to ensure identified needs are met.
• Coordinates individualized planning with clients to meet short- and long-term goals.
• Clerks case conferences, monitors progress, and assists clients to achieve goals.
• Advocates for participants before agencies and community bodies as needed.
