Job summary

VOALA is seeking a Case Manager Generalist to coordinate services for clients with low acuity in Los Angeles County. You will manage a diverse caseload and provide comprehensive, person-centered support to connect clients with housing, health, education, and community resources.

On-site position requiring active weekly engagement with clients and team members. Must complete VOALA Case Management training within 6 months and maintain robust documentation and advocacy throughout the case plan.

Qualifications

• Must be able to pass a fingerprint clearance and background check.
• Experience in case management or related social services.
• Completion of VOALA Case Management training within 6 months of hire.

Responsibilities

• Conduct screenings and assess needs including environmental and health factors.
• Develop and coordinate a plan of action to meet short- and long-term goals.
• Provide service linkages and support to ensure identified needs are met.
• Coordinate case conferences and advocate for clients with external entities.
