Job summary

VOALA is a Los Angeles-area nonprofit dedicated to helping vulnerable communities through services like Head Start, housing, and veterans’ programs. The Case Manager Specialist I provides intensive case management, meeting participants weekly and coordinating services as the primary contact.

On-site role requiring driving to client meetings and home visits, with strong advocacy and thorough documentation. Requires an Associate’s Degree or related experience and completion of VOALA Case

Qualifications

• Associate’s Degree in Social Work or related field, or combination of education and relevant experience (two years’ total).
• Completion of VOALA Case Management training within 6 months of hire.
• Minimum 1-year general experience in case management or similar social services role

Responsibilities

• Conducts comprehensive screenings and assessments to collect information to develop a case plan.
• Provide service linkages and support systems to ensure identified needs are met.
• Care Coordination, Case Management of assigned caseload.
• Coordinates individualized planning with clients to meet short- and long-term needs.
• Coordinates case conferences with clients and monitors progress of case plans.
• Must drive to client meetings and home visits on a regular basis.
• Advocacy for participants before other agencies/community settings.
• Documents and charts group education sessions clearly and concisely.
