Qualifications

• Must be qualified to independently complete an assessment within the scope of practice of one's discipline.
• Active license in the state; CCM/ACM certification required OR must be obtained within two years of start date.
• Minimum of 2 years rehabilitation experience preferred.
• RN licensure with BSN preferred or diploma acceptable in states where required; for other professionals, bachelor's degree is required.

Responsibilities

• Coordinate patient care from admission to post-discharge.
• Oversee interdisciplinary plan-of-care decisions.
• Participate in discharge planning and family conferences.
• Ensure accuracy of discharge and payor-related information in patient records.
• Maintain knowledge of regulations, standards, and department operations.
