Job summary

You will work with an interdisciplinary team to develop and adjust plans of care, monitor patient experience, and ensure timely discharges in collaboration with payors.

Qualifications

• Must hold active license if required for discipline
• CCM/ACM certification within two years of entry or already obtained

Responsibilities

• Coordinate discharge planning with interdisciplinary team
• Oversee plan-of-care decisions from admission to post-discharge
• Monitor patient experience and quality timeliness of care
• Ensure discharge and payor information accuracy in patient records
• Participate in utilization review and case management meetings
• Complete required documentation and addendums
