Encompass Health is seeking a Case Manager to link exceptional care with patient outcomes across our national rehabilitation network. You will manage resources, coordinate care from admission to post-discharge, and oversee interdisciplinary plan‑of‑care decisions to support meaningful patient journeys.

Qualifications

• Must be qualified to independently complete an assessment within the scope of practice.
• Active license in the state.
• CCM or ACM certification required OR to be obtained within two years of entry.
• Bachelor's degree required; graduate degree preferred for advancement.

Responsibilities

• Work with interdisciplinary team, guiding treatment plans based on patient needs.
• Coordinate with interdisciplinary team to establish tentative discharge plan and contingency plans.
• Participate in planning for and execution of patient discharge experience.
• Monitor patient experience: quality/timeliness/service appropriateness/payors/expectations.
• Facilitate weekly team conferences and coordinate treatment plan modifications.
• Complete case management documentation and addendums.
• Maintain knowledge of regulations and department procedures.
• Review/ analyze case management reports and act on KPIs.
• Attend ACT meetings to identify trends and reduce transfers.
• Meet with patient/family within 24 hours of admission and plan care within 48 hours.
