Job summary

Encompass Health is seeking a Case Manager in Cleveland, TN to coordinate patient care from admission through post-discharge, partnering with an interdisciplinary team to ensure quality outcomes. This role emphasizes care coordination, discharge planning, and regulatory compliance to enhance the patient journey.

You will operate within a national inpatient rehabilitation leader, with opportunities for growth, education, and involvement in performance improvement and patient experience

Qualifications

• Must be able to independently complete an assessment within the scope of practice.
• Must hold an active license in the state.
• CCM® or ACM™ certification required OR must be obtained within two years of entry.
• Bachelor's degree required for licensed or certified health professionals; graduate degree preferred.

Responsibilities

• Work with interdisciplinary team to guide treatment plans based on patient needs and preferences.
• Coordinate with team to establish tentative discharge plan and contingency plans.
• Participate in planning for and execution of patient discharge experiences.
• Monitor patient experience: quality, timeliness, service appropriateness, payors, expectations.
• Facilitate weekly team conferences and coordinate all treatment plan modifications.
• Complete case management addendums and required documentation.
