SupportWorks Housing, formerly Virginia Supportive Housing, seeks a dedicated Supportive Services Specialist for The Crossings at Fourth and Preston. You will work with a Team Lead to support low income and disabled adults who may be experiencing homelessness, performing assessments, treatment planning, counseling, and care coordination.

Responsibilities include outreach, engaging families and providers, advocating for clients, and maintaining detailed client records and discharge summaries.

Formación

• Bachelor's degree in human services preferred; 1+ year of direct service with adults with low income or housing issues is desired.
• Knowledge of homeless populations and community resources is preferred.
• SOAR certification and CPR/First Aid certification preferred.

Responsabilidades

• Provide case management for low income and disabled adults experiencing homelessness or related challenges.
• Assess, plan, provide supportive counseling, and link clients to community resources.
• Collaborate with other providers and law enforcement as needed and maintain client files and progress notes.
