You will provide coaching, education, and skill-building to help clients achieve housing stability and community integration while applying trauma-informed, person-centered approaches.

The role emphasizes landlord collaboration, benefits navigation, and data-driven progress updates to support wraparound services and successful discharge planning.

Qualifications

• Coaching and support skills for housing stability.
• Ability to educate and assist with housing programs and applications.
• Experience with trauma-informed and strengths-based approaches.

Responsibilities

• Provide coaching, education, support, and skill-building related to individualized housing stability and community integration goals.
• Deliver services using SAMHSA Evidence-Based Practices that support the fidelity model for Supportive Housing.
• Screen individuals for eligibility and complete Housing Needs Assessments.
• Assist with move-in coordination, ensuring participants have required supplies and resources.
• Educate and support participants with benefits applications, housing applications, SSI/DSHS documentation, and other referrals.
• Maintain data collection and reporting systems and ensure compliance with program guidelines.
