Job summary

Apis Services, Inc. is seeking a dedicated Case Manager to provide support to homeless adults with behavioral health challenges. This role involves outreach, service linkage, and management of case planning and assessment to ensure clients receive proper care.

The ideal candidate will possess at least a bachelor’s degree in a relevant field, strong communication, organizational skills, and must be adept at working both independently and collaboratively. Benefits include medical, dental, and vision coverage as well as generous PTO.

Qualifications

• Experience working with individuals with severe mental illness and/or substance abuse.
• Experience in data collection and evaluation.
• CPR and First Aid certification.
• Valid driver’s license with at least 3 years of continuous driving experience and a clean record.

Responsibilities

• Build relationships with clients and encourage community involvement.
• Ensure health, safety, and well-being of clients.
• Coordinate benefits applications and liaise with state agencies.
• Facilitate transitions between different care levels.
• Respond to crises and provide immediate services.
