Job summary

HELP USA is seeking a Case Manager for the Crotona program in the Bronx. You will connect clients experiencing homelessness with housing, coordinate with the service team to build independent living plans, and hold regular client meetings to drive progress toward stability.

Responsibilities include directing referrals to employment, education, and health services, and supporting clients as they work toward self-sufficiency in a compassionate, client-centered environment.

Qualifications

• Bachelor's degree in social work or related field or equivalent experience.
• Strong ability to manage multiple priorities and keep stakeholders informed of status.
• Excellent written and spoken communication skills.

Responsibilities

• Coordinate with the service team to develop independent living plans for clients.
• Meet with clients regularly to help implement their plans and secure housing.
• Provide referrals for employment, education, medical and mental health services, childcare, and substance use support.
