Job summary

Jericho Project in New York seeks a Case Manager to provide residential and long-term support to homeless and at-risk adults transitioning into permanent housing. You will manage a caseload of approximately 20–25 individuals, with frequent tenant contact, meetings, and aftercare contacts, including documentation in electronic health records.

The role requires strong organizational and communication skills, experience with low-income or homeless populations, and a preference for Spanish fluency.

Qualifications

  • Bachelor’s degree in human services or related field.
  • Strong organizational, writing, communication, and computer skills.
  • Some relevant experience working with low-income or homeless populations.
  • Fluent in Spanish is strongly preferred.
  • Candidates must be self-starters with energy to work within the community.

Responsibilities

  • Provide direct case management services on-site and in the field, including mental health and substance use assessments and service planning.
  • Use person-centered approaches and evidence-based models such as Motivational Interviewing, Critical Time Intervention and Harm Reduction.
  • Maintain accurate tenant files/charts in electronic health records and update case notes.
  • Develop and review Individual Service Plans and document progress toward goals.
  • Coordinate with community providers and serve as liaison with tenants.
  • Facilitate groups on life skills, education, relapse prevention and mental health counseling.
  • Complete referrals to medical, mental health, and other outside providers.
