Job summary

Bowery Residents' Committee in New York City seeks a dedicated Case Manager to provide comprehensive services to chronically homeless individuals. You'll manage a caseload, meet clients in the field and office, and ensure all files and reports are up to date, guiding clients through housing barriers and entitlements.

You will collaborate with clinical staff, participate in case conferences, and support clients through the Transit Homeless Outreach program across Manhattan, The Bronx, Brooklyn

Qualifications

• BA in related field preferred; significant outreach or case management experience.
• Ability to assess signs of mental illness and substance use; strong writing and communication skills.
• Valid driver's license; CPR/First Aid certification or willingness to obtain.

Responsibilities

• Provide comprehensive case management; meet clients regularly in the field and office; keep clinical files up to date.
• Identify housing barriers; assist with entitlements; progress individualized housing plans.
• Prepare clients for housing interviews; coordinate housing appointments.
• Liaise with clinical staff in other BRC programs and external partners.
• Participate in case conferences; perform related duties as assigned.
