Job summary

Benchmark Human Services seeks a Case Manager in Vermont to support individuals with disabilities. You will coordinate person-centered planning, assessments, and service planning with families and guardians while promoting independence and community inclusion.

The role involves traveling up to 75% within the state to provide resources and advocacy, ensuring plans align with the person’s goals and needs.

Qualifications

• Must know Vermont public service system for developmental disabilities and brain injury services.
• Able to work collaboratively to meet individual performance goals.
• Strong verbal and non-verbal communication with a broad audience.
• Ability to use technology tools in daily duties.

Responsibilities

• Perform evaluations and assessments to meet needs of individuals served.
• Support person-centered planning and assessment activities with the team.
• Develop agendas and follow-up notes for planning meetings.
• Monitor plan implementation and health and welfare through visits and reviews.
• Review and update service plans and related documents with the individual.
