Job summary

Benchmark Human Services is hiring Case Managers in Vermont to support individuals with intellectual and developmental disabilities and their families. You will plan, coordinate, and oversee medical, social, educational, and support services, helping clients participate fully in their communities.

Responsibilities include conducting evaluations, guiding person-centered planning, and updating service plans with the team. Travel up to 75% is required to meet client needs and complete assessments.

Qualifications

• Knowledge of Vermont's public service system for developmental disabilities services and brain injury services.
• Strong communication skills, including verbal and non-verbal.
• Ability to use technology tools in daily performance of duties.
• Ability to work collaboratively to meet individual performance goals.

Responsibilities

• Perform evaluations and assessments to meet the needs of individuals served.
• Assist individuals in understanding goals and activities of person-centered planning.
• Facilitate person-centered planning meetings and document follow-up actions.
• Review and update person-centered service plans with individuals and their teams.
• Monitor plan implementation and health and well-being through visits and coordination with guardians and providers.
