Job summary

Benchmark Human Services is hiring Case Managers in Vermont to support individuals with intellectual and developmental disabilities. The role involves coordinating resources, planning services, and assisting guardians to enable independent living and community participation.

We seek candidates with a bachelor’s degree plus relevant experience or an equivalent combination, state mobility, and willingness to travel up to 75%.

Qualifications

• Bachelor’s degree in any field + 1 year of human services related experience (supporting individuals with disabilities, special education, sociology, vocational rehabilitation, psychology, etc).
• Associate degree in human services or closely related field + 2 years of human services experience.
• High school diploma or equivalent + 3 years of human services related experience.

Responsibilities

• Perform evaluations and assessments to meet the needs of individuals served.
• Assist individuals to understand goals and participate in person-centered planning.
• Facilitate and schedule person-centered planning meetings and assessments.
