Benchmark Human Services is hiring Case Managers to support individuals with intellectual and developmental disabilities and their families in Vermont. You will guide planning, advocacy, and service coordination to help clients live independently in their communities.

Qualifications

• Must understand Vermont's public service system for DD and brain injury services.
• Experience communicating with diverse stakeholders in person-centered planning.
• Ability to document and track planning, assessments, and outcomes clearly.

Responsabilités

• Conduct core competency trainings as required by the State.
• Perform evaluations and assessments to support individual needs.
• Assist individuals in understanding goals within person-centered planning.
• Facilitate participation in planning meetings and scheduling.
• Complete annual Needs Assessment review with the individual and team.
• Guide discovery discussions to identify preferences, strengths, and goals.
• Help complete DS Budget and Service Planning Context documents.
• Coordinate additional assessments and ensure information is accessible to planning teams.
• Support the individual in selecting planning team members.
• Invite others to participate in planning meetings as appropriate.
• Develop planning agenda and distribute notes to the support team.
• Lead the individual through the planning process with support as needed.
• Identify modifications to rights and freedoms when required.
• Review and update the person-centered service plan with the individual.

Bachelor's degree + 1 year human services

Associate degree + 2 years experience

High school diploma + 3 years experience
