Job summary

Benchmark Human Services, a leader in IDD and behavioral health, seeks Case Managers to support individuals with disabilities and families in Vermont. You will coordinate planning, advocacy, and services to help individuals live independently and participate in their communities.

The role emphasizes person-centered planning, goal setting, collaboration with guardians and teams, and adherence to state requirements.

Qualifications

• Must complete core competency training required by the State.
• Performs evaluations and assessments to meet the needs of individuals served.
• Assist and support the individual to understand the goals and activities of the person-centered planning process.
• Facilitate and schedule participation in person-centered planning and assessment as needed.
• Complete Needs Assessment Annual Review Protocol once a year with the individual and their team.
• Assist the individual in discovery and exploration discussions, identifying preferences, strengths, goals and dreams, and needs in preparation for person-centered planning process.

Responsibilities

• Assist the individual to invite other people of the individual’s choosing to participate in person-centered planning meetings.
• Work with the individual to develop agenda(s) and follow up notes for meetings.
• Support the individual to lead, and provide assistance as needed, throughout the person-centered planning process.
• Review, assess and respond to emergency situations and incidents to protect health and welfare.
