Job summary

Benchmark Human Services in Vermont is seeking a Case Manager to provide resources and support to individuals with intellectual and developmental disabilities, their families, and guardians to promote independence and community inclusion.

You will coordinate planning, advocacy, and service delivery, facilitate person-centered planning, and complete assessments to ensure services align with the individual’s goals. Relevant human services experience and a related degree are preferred.

Qualifications

• Knowledge of Vermont’s public service system for developmental disabilities services and brain injury services.
• Understanding of state statutes, rules, and program requirements.
• Ability to work collaboratively while meeting individual performance goals.
• Ability to communicate effectively with a broad audience of stakeholders.
• Ability to use technology tools in daily performance of duties.

Responsibilities

• Perform evaluations and assessments to meet the needs of individuals served.
• Assist and support the individual to understand the goals and activities of the person-centered planning process.
• Facilitate and schedule participation in person-centered planning and assessment.
• Complete Needs Assessment Annual Review Protocol with the individual and their team.
• Assist the individual in discovery and exploration discussions, identifying preferences, strengths, goals and needs.
• Lead or support planning meetings and ensure distribution of notes to the support team.
• Monitor plan implementation and well-being in collaboration with guardians and family as applicable.
