Odyssey House in Salt Lake City is seeking a Full-Time Case Manager to support clients with comprehensive case management services. You will work closely with clinical staff to assess needs, coordinate care, document services, and ensure safety for clients in a fast-paced environment.

The role emphasizes collaboration with therapists, accurate record-keeping, and adherence to intake, documentation, and funding procedures while supporting client education and outreach activities.

Qualifications

• Bachelor's degree in a related field; two years of related employment or program graduate recommended for licensure.
• Willingness and ability to pass a criminal background check.
• First Aid and CPR Certification.
• Must comply with TB tests, flu and immunizations as required.

Responsibilities

• Interview and assess client needs and resources on caseload.
• Coordinate with licensed therapists to develop treatment plans and goals.
• Monitor outcomes and client compliance; maintain client records.
• Verify Medicaid funding eligibility and manage status during treatment episodes.
• Document all incidents and required reports; complete client documentation within 24 hours.
