Job summary

Odyssey House Inc. in Salt Lake City, UT seeks a Full-Time Case Manager to support clients with comprehensive case management and coordination with clinical staff.

Responsibilities include assessment, documentation, resource linkage, and Medicaid tracking to ensure successful treatment outcomes. You will work with licensed therapists to develop treatment plans, document services, and coordinate with community resources while maintaining client safety and progress.

Qualifications

• Bachelor's degree in a related field.
• Willingness and ability to pass a criminal background check.
• First Aid and CPR Certification (available at Odyssey House).
• Must meet employment eligibility and annual immunization requirements.

Responsibilities

• Interview and assess client needs and resources on case management caseload.
• Coordinate with licensed therapists in developing treatment plans and goals.
• Monitor outcomes and client compliance.
• Maintain client records and ensure paperwork is complete.
• Verify Medicaid funding eligibility and manage status during treatment episodes.
• Submit reports to supervisor as requested.
• Arrange client appointments and paperwork with community resources and follow up on progress.
• Complete service documentation within 24 hours in the client management system.
• Document services using ASAM Dimensional Analysis format with required elements.
• Proactively involve collateral supports (family, probation officers, etc.).
• Document incidents promptly and report as required.
• Prepare accurate client reports in required timeframes (e.g., court reports).
• Other duties as assigned.
