Responsibilities

Job description

Utah Case Management is looking to hire individuals who have experience as a Case Manager who have worked with low income, homeless individuals and justice involved individuals. The Case Manager assists with housing related services for the client through Utah Medicaid’s Target Adult Medicaid and Housing Related Service and Supports program. We offer a flexible schedule allowing you to govern your own hours. This is a hybridposition with remote documentation and in person client visits. We are looking for dedicated employees to assist with coordinating services for our clients.This job requires going into Correctional Facilities.
