Job summary

Humana Inc. invites you to join our Inclusa/Humana team as a Case Manager serving Wisconsin Family Care members in the Hayward/Park Falls area.

You will assess health and safety needs, create a comprehensive Member Care Plan (MCP), and coordinate with a Field Care Nurse to deliver services in the least restrictive environment. The role requires a Bachelor's degree in human services (or related field with relevant experience), a valid driver’s license, and the ability to travel up to 40%.

Qualifications

• Bachelor's degree in human services or related field with 1 year of experience serving frail elders or adults with disabilities.
• Bachelor's degree in another field with at least 3 years of experience serving frail elders or adults with disabilities.

Responsibilities

• Assess members health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to provide services in the least restrictive environment.
• Conduct face-to-face social assessments with members on enrollment and at least every six months.
• Conduct quarterly in-person visits and monthly phone contact with members.
• Arrange support services including social integration, community resources, housing, and non-medical needs.
• Ensure cost-effective service delivery.
• Evaluate risk factors and provide education to members.
• Maintain accurate documentation including case notes and MCP updates.

Education

Bachelor's degree in human services or related field
