Job summary

Humana Inc in Wisconsin is seeking a Case Manager to serve members in the Wisconsin Family Care program. You will assess health and safety needs, develop a comprehensive Member Care Plan, and coordinate with a Field Care Nurse to support members in the least restrictive environment.

Responsibilities include face-to-face assessments, regular visits, arranging non-medical services such as housing and community resources, and maintaining accurate documentation.

Qualifications

• Bachelor's degree in human services or related field with at least 1 year serving frail elders or disabled adults; or a Bachelor's in another field with 3+ years of such experience.
• Valid state driver's license and proof of personal vehicle liability insurance with 100/300/100 limits.
• Preferred: reside within 45 minutes of the assigned coverage area.
• Preferred: case management experience and electronic case note documentation.
• Preferred: knowledge of community health and social service agencies and resources.

Responsibilities

• Assess members' health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to address needs in the least restrictive environment per the MCP.
• Conduct face-to-face social assessments on enrollment and at least every six months.
• Conduct quarterly in-person visits and monthly phone contact with members.
• Arrange non-medical services, including social integration, housing, and community resources.
• Ensure cost-effective service delivery and maintain accurate documentation.
• Evaluate risk factors and provide member education.
