Job summary

Humana Inc. is hiring a Case Manager for the Inclusa/Humana team serving Wisconsin Family Care members in the Rice Lake/Luck area.

You will work under the Manager of Care Coaching to coordinate services for frail elders and adults with disabilities, delivering comprehensive case management and ensuring timely MCP updates. The role includes field coordination, home visits, and a focus on community resources, with travel up to 40% and a standard 40-hour work week.

Qualifications

• Bachelor's degree in human services or related field required.
• 1 year of experience serving frail elders or adults with intellectual, developmental, or physical disabilities, or 3 years in a related field with different degree.

Responsibilities

• Assess health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to address needs in the least restrictive environment.
• Conduct face-to-face social assessments at enrollment and at least every six months.
• Perform quarterly in-person visits and maintain monthly contact by phone.
• Arrange non-medical supports: social integration, community resources, housing, and employment.
• Ensure cost-effective service delivery and maintain documentation including MCP updates.
