Job summary

Humana Inc. is seeking a Case Manager to support Wisconsin Family Care members in the Hayward/Park Falls area. You will coordinate with care teams, conduct assessments, and develop the Member Care Plan to address health and social needs.

The role requires a bachelor's degree in human services or related field, or a related degree with relevant experience, plus valid driver's license and insurance. Travel up to 40% is expected.

Qualifications

• Must have a Bachelor's degree in human services or a related field with 1 year of relevant experience, or a Bachelor's in another field with 3 years of relevant experience.
• Valid state driver's license and proof of personal vehicle liability insurance with 100/300/100 limits.

Responsibilities

• Assess health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to address health and safety needs in the MCP and least restrictive setting.
• Conduct face-to-face social assessments at enrollment and at least every six months, usually at the member's residence.
• Conduct quarterly in-person visits and maintain monthly contact by phone.
• Arrange non-medical support services including social integration, community resources, employment, housing, etc.
• Ensure cost-effective service delivery.
• Evaluate risk factors and provide education to members.
• Maintain accurate documentation including case notes, service authorizations, and MCP updates.
• Use your skills to make an impact.
