Job summary

Humana Inc. in Wisconsin invites a skilled Case Manager to join the Inclusa/Humana team serving Family Care members. You will coordinate with nursing staff and deliver comprehensive, person-centered care for frail elders and adults with disabilities, primarily in the Juneau County area.

You will perform assessments, develop and monitor the Member Care Plan, arrange non-medical services, and maintain strong documentation while traveling to member homes as needed.

Qualifications

• Requires a Bachelor's degree in human services or related field.
• At least 1 year of experience serving frail elders or adults with IDD/physical disabilities.
• Valid state driver’s license and proof of insurance (100/300/100).

Responsibilities

• Assess health and safety needs and develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to provide services per MCP.
• Conduct face-to-face social assessments at enrollment and at least every six months.
• Conduct quarterly in-person visits and monthly phone contact.
• Arrange non-medical services (social integration, housing, employment).
• Maintain documentation including MCP updates.
