Job summary

Humana is hiring a Case Manager for the Wisconsin Family Care program in the Hayward/Park Falls area. You will assess health and safety needs, coordinate with a Field Care Nurse, and maintain the Member Care Plan. Remote work with travel up to 40% is required.

Requires a Bachelor's in human services or related field (or BA with 3+ years relevant experience) and a valid driver's license with insurance. TB screening applies.

Qualifications

• Bachelor's degree in human services or related field; or BA in another field with 3+ years relevant experience.
• Valid state driver's license and proof of personal vehicle liability insurance (100/300/100).

Responsibilities

• Assess members' health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to provide services per MCP in the least restrictive environment.
• Conduct face-to-face social assessments upon enrollment and at least every six months.
• Conduct quarterly in-person visits and monthly phone contact with members.
• Arrange non-medical support services (social, housing, resources).
• Maintain cost-effective service delivery and accurate documentation (case notes, authorizations, MCP updates).
