Job summary

Humana Inc in Wisconsin seeks a Case Manager for the Inclusa/Humana team serving Family Care members in the Rice Lake/Luck area. You will assess needs, develop MCPs, and coordinate with a Field Care Nurse to deliver services in the least restrictive environment.

You will conduct regular social assessments, quarterly visits, and monthly calls, arranging non-medical supports while ensuring cost-effective care and meticulous documentation. Remote/hybrid work with travel up to 40% is required.

Qualifications

• Bachelor's degree in human services or related field with 1 year of relevant experience or Bachelor's in another field with 3 years of relevant experience.
• Valid state driver's license and proof of personal vehicle liability insurance (100/300/100).
• Reside within 45 mins or less of assigned coverage area preferred.

Responsibilities

• Assess members health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to address needs per MCP.
• Conduct face-to-face social assessments at enrollment and at least every six months.
• Conduct quarterly in-person visits and monthly phone contact.
• Arrange social, housing, employment, and other non-medical supports.
• Ensure cost-effective service delivery and maintain documentation.
