Felton Institute is seeking a Bilingual Case Manager to support justice-involved transition-age youth (18-24) in San Francisco. You will work with the clinical team to build Wellness Care Plans, coordinate with the court, and connect clients to health, housing, and social services.

The role emphasizes culturally affirming practice, documentation accuracy, and collaboration with multiple agencies. Strong bilingual skills and experience with TAY populations are essential for success.

Formación

• Bilingual English/Spanish required.
• Bachelor’s degree in Social Work, Counseling Psychology, or related field with 3 years experience.
• AA degree plus 3 years or 5+ years working with TAY/justice populations acceptable.
• Ability to obtain jail clearance and LiveScan.
• Experience with mental health and co-occurring disorders helpful.
• Familiarity with harm reduction and referrals.

Responsabilidades

• Provide case management for justice-involved transition-age youth (18-24).
• Develop Wellness Care Plans with clinical staff and client input.
• Link participants to health, housing, benefits, and employment resources.
• Attend weekly group and court collaborations; maintain documentation.
• Represent the program in a culturally affirming, trauma-informed manner.
• Maintain data entry, generate reports, and ensure compliance.
