Job summary

OptumCare in Layton, Utah seeks an experienced MB I NCM to serve as a clinical liaison, coordinating care plans across the medical network. The role requires on-site visits, strong HIPAA conduct and a comprehensive care approach for high-risk members.

Responsibilities include home visits, CMS-compliant documentation, and collaboration with nurses, physicians and care coordinators to optimize health outcomes.

Qualifications

• Active, unrestricted RN license.
• 4+ years of nursing experience.
• Proficient in Word and basic computer skills.
• Clear communication with members, providers and the care team.
• Willingness to travel up to 75% for home visits.

Responsibilities

• Screen high-risk members and identify those who will benefit from care management.
• Perform member evaluations and onsite visits in multiple settings.
• Assess high-risk symptoms and co-morbidities to guide interventions.
• Document findings and develop individualized care plans compliant with CMS.
• Use clinical skills to address health, behavioral health, and psychosocial needs.
• Record status, diagnosis, medications, and progress in EMR timely.
• Advocate for members and families within the health care team.
• Participate in interdisciplinary team meetings with nurses and physicians.
• Coordinate with providers to determine acuity and refer to community resources.
• Support MD consultations on complex cases.
• Apply nursing/counseling/social work theory in practice.
• Manage assigned caseload efficiently.
